Background:Orlistat is the first-line medicine approved for the treatment of obese patients with overweight and obesity-related comorbidities. The main objective of this study was to compare the pharmacokinetics, safety, and pharmacodynamics of orlistat administered with placebo in overweight obese patients with obesity. Methods: A total of 60 overweight patients with an average body mass index (BMI) of 28.5 kg/m2 (25 to 29.9) were randomly divided into two groups. The first group was treated with orlistat (n=30) by intramuscular (IM) injection, and the second group received the same drug as the IM group. Pharmacokinetic parameters were evaluated before treatment, during the treatment period, and at the end of the study. Results: The pharmacokinetic parameters of orlistat in the first group were significantly lower than those in the second group at both initial (n=10) and final (n=30) time points. The pharmacokinetic parameters of orlistat in the second group were significantly lower than in the first group at initial (n=10) and final (n=30) time points. The pharmacokinetic parameters of orlistat in the third group were significantly lower than those in the first group at initial and final time points. Conclusions: Orlistat oral administration was more effective than placebo in the treatment of overweight obese patients with obesity. The study of Orlistat pharmacokinetics should be performed in a short-term (≥ 4 weeks) and longer-term (≥ 8 weeks) period to allow the long-term monitoring of the pharmacokinetics of the active ingredient and to allow an early initiation of drug therapy.

All patients received anastrozole (10 mg, 0.15 mg/kg) on the first day of the study, and all patients received the same dosage as with orlistat oral.
